University Student Health Insurance RequirementsRequirements for Waiver
Application for waiver from the university student health insurance plan must be submitted to UHS by the last day of the open enrollment period. Students applying for waiver must provide documentation of:- Coverage by a carrier that has offices based in the U.S.
- Continuing coverage that verifies enrollment as the dependent, partner/spouse or principal in an employer, state or federally operated insurance Exchange or government sponsored insurance plan.
- Enrollment in a plan that meets certain minimum standards for coverage as set forth by the university including but not limited to an adequate level of coverage for medical, mental health and substance abuse services at the primary, emergency, inpatient and outpatient level in the geographic location of study
- is provided by a company licensed to do business in the United States, with a U.S. claims payment office and a U.S. phone number;
- has unlimited annual and lifetime benefits (i.e., no policy maximum);
- will be in force for the duration of the semester;
- has a maximum deductible of $2,500 individual/$5,000 family. If the student¡¯s deductible is greater than $2,500/$5,000, the student must have a Healthcare Savings Account (HSA) that will allow the student to seek needed medical and mental health care that covers all deductible expenses over $2,500/$5,000;
5. covers all of the following (a policy that covers only emergency or urgent care does not meet this requirement): - Routine, urgent and emergent care for both inpatient & outpatient medical care;
- Mental health treatment;
- Alcohol & substance abuse treatment;
6. Health Maintenance Organization (HMO) plans are acceptable as long as the student is physically located in the provider network service area or has an ¡°away from home¡± rider or guest membership; 7. Medicaid plans are acceptable as long as student is physically located in the same state that provides the Medicaid Coverage. (Examples: Acceptable: Student residing in FL and has FL Medicaid. Not Acceptable: Student residing in PA and has FL Medicaid); and 8. provides coverage for pre-existing conditions with no waiting period or limitations.
